Prostate cancer: The challenge of comparing open and laparoscopic surgery
Karim Chamie and Mark S. Litwin
p121 | doi:10.1038/nrurol.2010.21
A recent study using claims data to compare open retropubic with minimally
invasive prostatectomy has revealed higher rates of genitourinary
complications, incontinence and erectile dysfunction after the laparoscopic
procedure. However, there are a number of inherent problems associated
with the use of claims-based diagnoses for the evaluation of surgical
techniques.

Prostate cancer: Worse sexual function after cryoablation-a case for
EBRT?
Mack Roach III
p122 | doi:10.1038/nrurol.2010.11
External beam radiation therapy and cryoablation can both be used to
treat localized prostate cancer. Quality of life outcomes are similar
with the two forms of treatment, with the exception of sexual function,
which is poorer in patients treated with cryoablation than in those who
receive radiation therapy.

Kidney cancer: First guidelines for unilateral cT1 renal tumor management
Frederik C. Roos and Joachim W. Thuroff
p124 | doi:10.1038/nrurol.2010.22
Current treatment options for clinical stage T1 renal tumors, in patients
with a contralateral healthy kidney, are diverse. The first guidelines
for the management of these lesions have recently been published, which
state that tumor size and health status of the patient are important
factors to consider. The inclusion of tumor location, patient age and
major comorbidities might strengthen these guidelines in the future.

New treatments for chronic prostatitis/chronic pelvic pain syndrome
Adam C. Strauss and Jordan D. Dimitrakov
Published online: 09 February 2010
p127 | doi:10.1038/nrurol.2010.4
The etiology and pathogenesis of CP/CPPS remain unknown, but it is becoming
clear that neuropathic pain plays an important role. Here, Strauss and
Dimitrakov discuss emerging treatments that are known to target pain
pathways, and explore the implications of these treatments with respect
to unraveling the etiology of CP/CPPS.

Clinical implications of changing definitions within the Gleason grading
system
Tamara L. Lotan and Jonathan I. Epstein
Published online: 16 February 2010
p136 | doi:10.1038/nrurol.2010.9
Since its inception in 1966, the Gleason system for the grading of
prostate cancer has evolved to keep pace with changing practice in both
pathology and urology. Here, Lotan and Epstein discuss the changes to
the definition of pattern 3 and 4 disease, and highlight the impact of
these modifications on research and clinical practice.

Emerging gene and stem cell therapies for the treatment of erectile
dysfunction
Ahmed Harraz, Alan W. Shindel and Tom F. Lue
Published online: 16 February 2010
p143 | doi:10.1038/nrurol.2010.8
Treatment aimed at restoring the underlying tissue and vascular defects
that cause erectile dysfunction is sorely lacking. Harraz et al. describe
recent advances in the use of gene therapy to improve penile function,
highlighting the potential therapeutic benefits. The current status of
stem cell research in this area is also discussed.

The effect of obesity on sperm disorders and male infertility
Stefan S. Du Plessis, Stephanie Cabler, Debra A. McAlister, Edmund Sabanegh
and Ashok Agarwal
Published online: 16 February 2010
p153 | doi:10.1038/nrurol.2010.6
Obesity is associated with an increased incidence of male factor
infertility. By synthesizing new and old data, the authors of this Review
offer a completely novel perspective on the relationship between high
BMI, disordered spermatogenesis, and fertility problems. Potential
mechanisms and available treatment options are succinctly discussed.

Jens Sonksen
Published online: 16 February 2010
p162 | doi:10.1038/nrurol.2010.7
Men with spinal cord injury (SCI) have severely impaired fertility,
characterized by erectile dysfunction, ejaculatory dysfunction, and
semen abnormalities. In this Review, authors from the US and Denmark
