Highly active antiretroviral therapy (HAART) has a variety of adverse renal
effects. Patients receiving HAART can develop acute kidney injury, which
can progress to chronic kidney disease. HAART also contributes indirectly
to kidney disease by increasing the risk of diabetes and hypertension.
This Review describes the epidemiology, mechanisms and diagnosis of
HAART-related kidney damage, with an emphasis on the preventive management
of risk factors such as intravascular volume depletion and pharmacological
interactions.

to transplantation because of the risks associated with immunosuppression
in patients who are already immunocompromised, the lack of long-term

colleagues examine the issues surrounding kidney transplantation in patients
with HIV and describe a range of strategies that optimize outcomes in

and 2.6 million people in sub-Saharan Africa could have HIV-associated

disease to set the scene for a detailed analysis of this issue in Africa.
The authors proffer advice on how to prevent or slow progression of renal
disease and outline options for renal replacement therapy.

This case study discusses a patient initially diagnosed with high-risk
prostate cancer who underwent androgen deprivation therapy. The patient
subsequently presented with ductal carcinoma in situ and was found to
be a BRCA2 mutation carrier with a strong family history of breast cancer.

Molecular imaging of solid tumors: exploiting the potential

Novel targeted therapies often result in disease stabilization rather
than tumor shrinkage, which are reflected by changes in the molecular
features of the tumor. Noninvasive procedures to measure such features
are urgently needed. The authors of this opinion article discuss the

cytotoxic and targeted therapy.

The glucocorticoid receptor (GR) evolved more than 400 million years
ago from an ancestral receptor that was activated by a trio of
hormones (cortisol, aldosterone and deoxycorticosterone).
Researchers have now shown that mutations that impart selectivity to
cortisol cannot be reverted; suggesting that changes in protein
function or structure happening over the course of tens of millions
of years can prevent molecular turnarounds and render evolution
irreversible.
